The initial assessment is much more productive if basic medical information is prepared by you in advance. In order for you to get the most out of your appointment, and in order for the physician to provide you with a full assessment, a completed form is imperative.
Please fill out the form below and bring it with you to your appointment.
If this is not possible, please plan to arrive 30 minutes early to complete it at the Centre. Please note that all forms have to be completed prior to meeting with Dr. Nahas.

What to bring
Your assessment will be easier and more productive if you come prepared. Please bring a notebook or binder. Every person with an illness should keep their health-related information in one place. You may want to use the notebook to write down what physicians say, to record test results, keep track of symptoms and write down questions that arise - these are often forgotten during appointments.
We need to know your medical history in as much detail as possible. This includes:
- actual bottles of current medications and supplements you are taking, and the reason they were prescribed
- dates of operations, reason for the surgery, the outcome and any complications you experienced
- a list of other physicians you have seen or are seeing - and what they have told you about your illness
- recent blood test results
- reports of CT, MRI or other imaging tests (actual images not required)
- a list of treatments you have been through in the past - dates, number or duration of treatment, how successful they were
Try and recall the details of your life at the time your illness began - and other illnesses you suffered in childhood. Talk about this with your family members to help you remember as much as possible. Minor illnesses, life changes, injuries or traumas may have an impact on your health that you would not have expected.
The time you spend preparing for and thinking about your assessment will help us determine what you need, and it can be rewarding in unexpected ways. The process of examining yourself and your life is an important part of the healing process.
What you need to know about the initial consultation
You can expect to spend a minimum of one hour at your integrative assessment. During your visit, Dr. Nahad will perform a detailed history and physical examination. This will include current health concerns and past medical issues, including diagnostic testing and conventional and alternative medical treatments.
Your patient intake form and timeline will form the basis of this encounter, so please take the time to fill it out carefully.
Following this initial evaluation, Dr. Nahas will synthesize and interpret all the information to identify the underlying problems that may be affecting your health and establish a comprehensive plan for diagnosis and treatment.
Most patients are further evaluated with diagnostic tests. There is no fee for insured diagnostic tests. If specialized testing is required, they will be at an additional cost. These will be arranged after your appointment or at another time.
